RESUMO

Objetivo: Analizar la eficacia terapéutica, seguridad y valor pronóstico de diferentes biomarcadores de la radioembolización transarterial con esferas de itrio-90 (TARE) en pacientes con metástasis hepáticas de cáncer colorrectal. Material y métodos: Estudio prospectivo que incluye los pacientes con metástasis hepáticas de cancer colorrectal tratados con TARE entre noviembre de 2015 y junio de 2020. Se analizó la respuesta terapéutica (3 y 6 meses, criterios RECIST v1.1) mediante el cálculo de las tasas de respuesta tumoral objetiva (ORR) y de control de la enfermedad (DCR), así como la asociación de los biomarcadores con la respuesta terapéutica y la supervivencia global (SG) y libre de progresión (SLP). Resultados: Treinta TARE en 23 pacientes (edad media 61,61±9,13 años; 56,5% varones). La ORR a los 3 meses fue del 16,7% y el DCR del 53,3%. A los 6 meses progresaron el 80% de los pacientes. La ORR y DCR se asociaron con la edad (p=0,047), tratamiento con bevacizumab (p=0,008), hemoglobina (p=0,008), NLR (p=0,040), albúmina (p=0,012) y GPT (p=0,023) previas a la TARE, y la dosis absorbida tumoral estimada>115Gy (p=0,033). La mediana de SG fue de 12 meses (IC 95%: 4,75-19,25 meses) y de SLP 3 meses (IC 95%: 2,41-3,59 meses). La SG se asoció con la cirugía del tumor primario (p=0,019), mutación KRAS (p=0,024), hemoglobina (p=0,009), NLR (p=0,005) y PLR (p=0,042) previos a la TARE. Conclusión: Los biomarcadores con capacidad para predecir el pronóstico y respuesta terapéutica a la TARE incluyen desde parámetros bioquímicos a factores relacionados con la dosimetría tumoral estimada (AU)

A 55-year-old man with post-traumatic central cord injury, diagnosed with remitting seronegative symmetrical synovitis with pitting edema (RS3PE syndrome). The clinical picture begins with an acute pain in hands, swelling and stiffness. The physical examination revealed edema with fovea on the back of both hands, with pain and swelling in the metacarpophalangeal joints. Given this, different analytical tests and radiography of hands were requested. We started treatment with 30mg of prednisone, showing significant clinical improvement, disappearing arthritis and edema, and normalization of the analytical values. The peculiarity of presentation of RS3PE syndrome in a patient with a central cord injury is due to the difficulty of identifying it due to the superposition of clinical manifestations together with the lack of knowledge of it, being in the absence of this lesion, an easily diagnosable pathology, do not require excessive complementary tests, and with an excellent prognosis after the appropriate early treatment.

RESUMO

Varón de 55 años con lesión centromedular postraumática, diagnosticado de sinovitis simétrica seronegativa remitente con edema con fóvea (remitting seronegative symmetrical synovitis with pitting edema o síndrome RS3PE). El cuadro clínico comienza con dolor agudo, hinchazón y rigidez de manos. La exploración física muestra edema foveolar en el dorso de ambas manos, dolor y tumefacción en las articulaciones metacarpofalángicas (MCF). Ante esto, se solicitan diferentes pruebas analíticas y radiografía de manos. Iniciamos tratamiento con 30mg de prednisona, evidenciando mejoría clínica significativa, desapareciendo la artritis y los edemas, y normalización de los valores analíticos. La peculiaridad de presentación del síndrome RS3PE en un paciente con lesión centromedular se debe a la dificultad de su diagnóstico por la superposición de manifestaciones clínicas junto al desconocimiento del mismo, pudiendo ser, en ausencia de esta lesión, una enfermedad de fácil diagnóstico, al no requerir excesivas pruebas complementarias, y con un excelente pronóstico tras el tratamiento precoz adecuado

Justificación y objetivos: El precepto islámico del Ramadán (R) impone a las personas que lo practican, importantes modificaciones fisiológicas y psicológicas, debido a las restricciones hídricas y dietéticas a las que se ven sometidos durante las horas diurnas a lo largo de un mes, estableciendo un modelo intermitente de ayuno absoluto, de naturaleza singular, particularmente cuando éste se lleva a cabo en sociedades multiculturales de tipo occidental, en las que no se producen ajustes horarios en las actividades diarias, que sí son habituales en los países de mayoría musulmana. Entre las modificaciones, por esta causa, destacan la activación de mecanismos de adaptación a la restricción hidrosalina, con consecuencias en la homeostasis de agua e iones plasmáticos. Por la relevancia de la cuestión y ante el escaso conocimiento de los efectos del (R) sobre el equilibrio iónico, se plantea como objetivo de este estudio, el análisis del comportamiento del ión potasio durante este mes de ayuno, con el fin de prevenir algunos problemas que pudieran afectar a la salud. Metodología: Se seleccionaron 10 jóvenes musulmanes, varones, sanos y con edades entre 18 y 25 años que realizaron el R y se analizaron parámetros bioquímicos e iones en sangre y orina, así como niveles plasmáticos de Renina y Aldosterona, una semana previa al R, primera y cuarta del periodo de ayuno y una semana después de finalizado éste. Resultados: Durante el mes del R, se produce, durante la mañana, un descenso de la excreción de potasio en orina lo que origina incrementos en la concentración plasmática de potasio; a lo largo de la tarde tiene lugar un aumento de su excreción que resulta más eficaz durante la cuarta semana del R. Discusión: Los cambios experimentados a nivel tubular que afectan a la disponibilidad de Na+, HCO3 - y pH, por efecto del R, parecen estar limitando la excreción del ión, a pesar de que se mantiene estimulado el sistema Renina-Aldosterona a lo largo del ayuno. Conclusión: Estos hallazgos ponen de manifiesto la necesidad de estudios específicos sobre la homeostasis de K+ durante el R, para dilucidar qué factores y mecanismos están determinando los incrementos observados en los niveles plasmáticos del ión (AU)

The consequence of this fact is the increase of the levels in plasma concentration of potassium; throughout the afternoon, an increase in its excretion results more effective during the fourth week of R. Discussion: Changes in the availability of Na+, HCO3- and variability of pH, in the distal tubule, appear to be responsible for the potassium limited excretion observed during the fast of Ramadan. Conclusion: These finding put into consideration the need of further studies focussing on potassium homeostasis during Ramadan in order to determine which factors are implicated in the raised levels of K+ observed in plasma (AU)

BACKGROUND AND OBJECTIVE: The aim of the present study was to compare the risk stratification obtained with the Framingham function, recommended by the National Cholesterol Education Program (ATP-III), and the risk charts of the Systematic Coronary Risk Evaluation (SCORE) program, in a cohort of subjects between 20 and 75 years (mean 59 years, 70% females) with metabolic syndrome. SUBJECTS AND METHOD: Participants were classified as low, moderate, or high risk by ATP-III and by SCORE of risk at 10 years, respectively. RESULTS: 8.3% of males and 7.4% of women from our study were classified as high risk in 10 years time by Framingham-DORICA. Regarding SCORE program 50% men and 29.6% of women were considered to have high coronary risk. CONCLUSIONS: Risk estimation with SCORE and Framingham function shows significant gender and quantitative differences. There is no concordance between Framingham and SCORE function to estimate cardiovascular risk on subjects included on the high-risk group.

This fact makes a singular constrat, particularly in multicultural Western societies, where there is no schedule adjustments in daily activities, which are common in predominantly Muslim countries. Among the changes, includes the activation of mechanisms of adaptation to the hydrosalin restriction, with consequences on the homeostasis of water and ion plasma. On the relevance of the issue and the limited knowledge of the effects of (R) on the ion balance, the aim of the present study is to analyze the behaviour of potassium ion for this month, in order to prevent some problems that may affect health. La práctica del precepto islámico del Ramadán (R), impone a las personas que lo practican importantes modificaciones fisiológicas y psicológicas debido a las restricciones hídricas y dietéticas a las que se ven sometidos, lo que tiene una especial repercusión en el rendimiento físico e intelectual de los estudiantes musulmanes, particularmente en sociedades multiculturales de tipo occidental, en las que no se producen ajustes horarios, en las actividades diarias. Entre las modificaciones introducidas en los hábitos de vida, por esta causa, destacan los cambios dietéticos, tanto cualitativos como cuantitativos. Por la relevancia de la cuestión y ante el escaso conocimiento de los efectos del Ramadán en estudiantes adolescentes que viven en sociedades no islámicas, se plantea como objetivo de este estudio, el análisis de las modificaciones en macronutrientes, ocurridas durante el R, en un grupo de alumnos musulmanes de ambos sexos, con el fin de solucionar y prevenir algunos de los problemas que se plantean en este periodo. Material y método: Se seleccionaron 55 jóvenes musulmanes (24 hombres y 31 mujeres) con edades comprendidas entre 15 y 18 años, que realizan el R y se analizó la dieta mediante inventario de 3 días, una semana antes del ayuno y en la segunda semana durante éste. Resultados: En el R, no hay cambios significativos en la ingesta calórica de los hombres y sí en el de las mujeres. Se incrementa el consumo de carbohidratos y proteínas y desciende el de grasas. Discusión: Los cambios en R conducen a una dieta más ajustada a las recomendaciones nutricionales, y a esto parece haber contribuido el hecho de que los participantes llevaron a cabo sus comidas en el seno doméstico donde, a pesar del corte occidental de la población, se deja sentir una importante influencia de las tradiciones y hábitos magrebíes, por el contacto fronterizo con Marruecos (AU)

Due to the importance of this issue and facing the limited knowledge the Muslim teenagers who live in non-Islamic societies have about the effects of Ramadan, it is suggested as main objective of this study the analysis of changes in nutrients during R in a group of Muslim students of both sexes, in order to solve and prevent some of the problems shown in this period. Materials and methods: 55 Muslim youngsters were selected (24 men and 31 women) aged between 15 and 18, who practise R; and the diet was analysed by inventory over 3 days, one week before fasting and in the second week during it. Results: In R there are no relevant changes in caloric intake. The consumption of carbohydrates and proteins is increased, and that of fat is decreased. Discussion: the changes in R lead to a diet more regulated to nutritional recommendations; and to this seems to have contributed the fact that the participants took their meals at home, where, in spite of the Western cut of the population, it is felt an important influence of North African traditions and habits, due to its border contact with Morocco (AU)

En España el número de Interrupciones Voluntarias de Embarazo (IVE) es elevado, así como la tasa de embarazos no deseados. en los últimos años ha variado notablemente el abanico de métodos anticonceptivos disponibles en nuestro país. Algunos de ellos, ya disponibles en otros países, se han introducido aquí, como es el caso del implante hormonal subcutáneo (IHS), considerado en la actualidad el anticonceptivo más eficaz, presentando además un excelente balance riesgo/beneficio. En el presente trabajo se analiza el perfil de las usuarias, así como los efectos adversos, grado de satisfacción e indicación del implante hormonal subcutáneo (IHS)


In Spain, the number of Volutary Interruptions of Pregnancy is high, as well as the rate of unintended pregnancies. Recently, the range of contraceptive methods available for Spaniards has widened considerably. Some of them, which were already available in other countries, have been introduced in Spain, such as the Subcutaneous Hormoonal Implant, regarded as the most effective contraceptive nowadays. Furthermore, it shows a great risk/benefit balance. The aim of this paper is the analysis of the users profile, the adverse effects, the degree of satisfaction and the methos used for women to carry the subdermal hormonal implant

RESUMO

Fundamento: Los cambios de hábitos durante el Ramadán, obligan a importantes reajustes fisiológicos, para mantener la homeostasis. El objetivo del presente trabajo ha sido analizar el perfil secretor de gastrina durante el ayuno de Ramadán en un grupo de musulmanes voluntarios, en edad escolar (13 a 15 años).Método: La muestra estuvo constituida por17 varones sanos, sin problemas gastrointestinales previos, seleccionados al azar entre todos los alumnos voluntarios de centros de enseñanza secundaria de Ceuta que practican el ayuno del Ramadán. Los niveles hormonales de gastrina se determinaron mediante radioinmunoanálisis. Resultados: Nuestros resultados muestran una disminución significativa de los niveles de gastrina al avanzar el período de ayuno, alcanzando el día 21, cifras de 23,89 pmol/L en comparación con los 28,49 de la semana previa(p=0,006) y de los 28,13 de la primera semana de ayuno (p=0,015). Estos cambios pueden interpretarse como una respuesta homeostática de adaptación a las nuevas pautas de alimentación. Finalizado el tiempo de Ramadán, observamos un incremento en los niveles de gastrina que devuelve los niveles hormonales a 30,15pmol/L, similares a los encontrados las semanas previa y primera del ayuno. Conclusión: Estos resultados sugieren un perfil de secreción para la hormona dependiente de la ingesta, que requiere un tiempo de adaptación a la nueva situación (AU)


Background: The changes in habits during Ramadan result in significant physiological readjustments, in order to maintain homeostasis. The main objective of the present study was to analyze the gastrin secretion during Ramdan Fast in a young muslim group during the school period between 13 - 15 years old. Method: Sample size was integrated by 17healthy men, without digestive personal history, selected by a randomized program between all the muslim students of the High Schools from Ceuta that practise Ramadan fast. Gastrin levels were determined by radioimmuno analysis(RIA).Results: Gastrin levels tend to go down during the fast period and at the 21st the media is23,89 pmol/L comparing with 28,49 pmol/L of the previous week (p=0,006) and the levels of the 1st week (28,13 pmol/L; p=0,015). These changes can be interpreted like an homeostatic answer of adaptation to the new guidelines of feeding. When Ramadan Fast concludes, gastrin levels come up again to basal levels (30,15pmol/L).Conclusion: These results suggest a secretion profile for the dependent hormone of the ingestion, that requires a time from adaptation to the new situation (AU)

RESUMO

El objetivo de este estudio ha sido conocer el estado nutricional de los ancianos de una residencia geriátrica de titularidad pública en la provincia de Granada, en el momento de su ingreso en la misma. Se estudiaron 50 residentes, durante los meses de enero-mayo de 2002, siendo excluidos del estudio los ancianos que presentaban deterioro cognitivo y todos los que procedían de otras instituciones. Para evaluar el estado nutricional se utilizó el "Mini Nutritional Assessment" (MNA).La prevalencia de desnutrición en nuestra muestra, fue del 11,1 por ciento en los hombres y del 18,8 por ciento en las mujeres, alcanzándose el 25 por ciento en el cuartil de 84-86 años, mientras que no presentó desnutrición el cuartil de 60-78 años. Nuestros resultados ponen de manifiesto la situación de riesgo nutricional en que se encuentran el 55,6 por ciento de los hombres y el 53,1 por ciento de las mujeres que ingresan en la residencia, no existiendo diferencias significativas para el riesgo nutricional entre hombres y mujeres. Estos datos sugieren que, sin una intervención nutricional adecuada, un número importante de estos ancianos en riesgo, pasarán a desarrollar durante su estancia en la residencia, cuadros de malnutrición (AU)

RESUMO

Justificación: el Ramadán se caracteriza por un período de ayuno acompañado de restricción hídrica durante el día, cambiando la ingesta de sólidos y líquidos a la noche durante un mes lunar. El propósito de este estudio ha sido valorar en un grupo de escolares adolescentes, la influencia del ayuno del Ramadán en el equilibrio hidrosalino. Metodología: para ello se seleccionaron de forma aleatoria un grupo de adolescentes escolarizados en centros multiconfesionales de la Ciudad Autónoma de Ceuta, que tras ser convenientemente informados aceptaron participar voluntariamente en el estudio. Se han analizado parámetros fisiológicos como peso, talla, frecuencia cardíaca, presión arterial, electrolitos en plasma y orina, osmolaridad, volumen plasmático y concentraciones hormonales de renina, aldosterona y vasopresina o ADH. Dichos parámetros se determinaron antes (A), durante (B y C) y después (D) del Ramadán. Resultados: los valores de aldosterona disminuyeron en la primera semana del Ramadán (B) y se elevaron considerablemente en las siguientes (A: 410,6ñ47,65; B: 278,2ñ27,60; C: 528,12ñ50,37 y D: 578,12ñ78,24 pg/mL), mientras que la ADH no se modificó significativamente en la primera semana de ayuno (B), disminuyendo en las posteriores (A: 9,1ñ0,78; B:8,9ñ0,79; C: 5,65ñ0,08 y D: 6,22ñ0,32 pg/mL). Los niveles plasmáticos de Na+ y K+ y sus fracciones de excreción mostraron variaciones coherentes con las de dichas hormonas. Conclusiones: la hiperhidratación debida a la ingesta excesiva de agua antes de salir el sol, provoca una hemodilución con disminución de la osmolaridad y mantenimiento del volumen plasmático y presión arterial (PA) a expensas de la inhibición de vasopresina o ADH y de Renina. El aumento en los niveles de K+ plasmáticos, sería responsable de la secreción significativamente elevada de aldosterona (p<0,05), observada el día 21de ayuno (C) y del aumento concomitante de la fracción de excreción de K+ (p<0,05) (AU)
